1.NOTICE STATEMENTS FOR ROUTINE TESTING

I understand that I am taking part in a routine testing programme to check whether I have the Covid-19 virus and I will provide swabs when requested.

All employees and contractors who are working on a site will be routinely tested if:

  • the site is on statutory Outage;
  • a Covid-19 outbreak has occurred on the site, as defined by Public Health England or Public Health Scotland (as applicable to the site); and/or
  • if the clinical risk level at the site is sufficiently high to warrant it (in accordance with EDF Energy processes).
  • Routine testing will help EDF Energy to minimise the spread of the disease on site from people who are a-symptomatic
  • I understand that this is a self-test swab process and that I should follow the instructions included in the pack.

Personal Data:

Personal Data will be collected from you during the testing process by EDF Energy and its 3rd party testing providers, and will need to be reported to Public Health England and/or Public Health Scotland (as applicable to the site) in the event of a positive test result. Next Steps:

If you have a positive test result:

  • you will be informed of your results by telephone or your nominated email address;
  • your line manager will also be notified;
  • you must self-isolate at home for 10 days;
  • you will be asked to provide details of the people with whom you have been in recent contact, so that they may be notified for track and trace purposes; and
  • your sample will be sent so that the virus itself can be genetically sequenced to ascertain which variant of Covid is present.
  • Public Health England and/or Public Health Scotland (as applicable to the site) will be notified.
  • If you have a negative test result, you will be informed of your results by text message and you may continue to work as normal.

 

  1. NOTICE STATEMENTS FOR TRACK & TRACE TESTING

Track & Trace Testing:

I agree to having a swab taken of the back of my throat and nose to test whether I have the Covid-19 virus because I am symptomatic or have been in contact with someone who may be positive, and that this swab can be self-administered under supervision or assisted if I would prefer.

I understand that I can choose not to be tested but that if I do so, I may be required to refrain from attending an EDF Energy site for 10 days, or to work greater than 2 metres away from all individuals for 10 days, depending on the assessed level of risk posed to the site.

[NB. This may be verbal consent from the individual and OH staff tick the box on screen to record it, so as to avoid transmission risk.]

  • If you have a negative test result, you will be informed of your results by the EDF Energy Occupational Health team, who will provide details about your return to work.
  • If you continue with this process  it is assumed that you have read and understood the information above and are happy to proceed with the testing on this basis.
